This exists to empower New York residents, families, and caregivers with the knowledge, clarity, and confidence needed to successfully navigate the NY Independent Assessor (NYIA) Uniform Assessment System (UAS) process. Through expert-led virtual coaching, we help individuals understand eligibility requirements, prepare for assessments, and communicate their functional and medical needs clearly—so no one is denied long-term care services due to misunderstanding or lack of preparation.
🌿 Patient and Caregiver Assessment Preparation Session
$125 – 75 minutes (Virtual via zoom)
Best for consumers preparing for an upcoming assessment.
Includes:
Overview of what to expect during the assessment
Guidance on how to describe daily functioning and needs
Review of common assessment questions
Anxiety-reduction coaching
Coaching on caregiver input during assessments
Education on how caregiving tasks are evaluated
Guidance on documentation and communication
Strategies to reduce caregiver stress
📋 Post-Assessment Review & Support
$125 – 75 minutes
For consumers or caregivers, after the assessment is completed.
Includes:
Review of the assessment experience
Clarification of next steps and timelines
Support with understanding outcomes (without interpretation or guarantees)
Planning questions for providers or care teams
